The Head Surgeon was once a Head of Staff who managed Medbay. The job was removed because it was mostly useless, and afterwards, the Research Director temporarily managed the medical section of the ship. Several features of Doctoring changed and the Medical Director took over.
The Head Surgeon job was brought back as a possible gimmick role in early 2019. They spawned with their favorite scalpel & circular saw combo, a set of scrubs instead of a crisp suit, and the same access as the Medical Director, with less authority and even less respect than before. Months later, it was retired again, presumably for still being just as useless as in the past.
Currently, a Head Surgeon spawns in medbay, either as a cardboard box or a medibot who says lines through Text-to-Speech.
Coincidentally, HeadSurgeon was the also name of Goonstation's IRC bot, which made funny replies to people who address it by name. Its silly quotes can be found here.
