This is the original item/inventory sprite for the medical eyepatch, yarr! This is specifically what it looked like when it was covering your right eye. (If you look at it from the front, the patch part is on the left, but from the view of the person wearing it, it's on the right.)
This image is not 32x32, but 24x12; it has been cropped to eliminate as much empty space as possible. Keep in mind, this might look a little weird if you put it with 32x32 sprites.
