Welcome, employee UNKNWNUSR, to the NanoTrasen Medical Database for the NT13, Atlas Star System. Here, you may browse for or add additional entries for personnel serving aboard.

Instructions

There are several fields in the table which, in the interests of maintaining standardization, should be filled out as such.

  • Surname and First name: Enter the surname and first name of the patient in these fields. In the case of more unusual naming schemes (such as Argonian-style names, eg. "Speaks With Stars"), the final word in the name is the surname while the rest encompasses the first name field. For example,

  • Nickname/Alias: Enter the aliases/nicknames of the patient in this field. In the event a character has two names, their primarily used name or the one that most closely matches conventional human naming standards should be put in the surname and first name fields.
  • Age: Enter the patient's age in standard solar years.
  • Gender: Enter the patient's preferred gender identity, which may not necessarily reflect their biological or physical appearance. The format for these are:
    • Male: M
    • Female: F
    • Other: O
  • Species: Enter the patient's species. The standard format for species names is as follows.
    • Humans: Human
    • Lizards/Saurians: Saurian
    • Roaches: Roach
    • Cow people: Bovine
    • Skeletons: Skeleton
  • Blood type: Enter the patient's blood type.
  • Puritan?: If the patient has the puritan trait, enter a "Y". If they do not, enter a "N". If unsure, enter a "?".
  • Borg?: Reflects the patient's acceptance to being borged. If they are willing, enter a "Y". If not, enter a "N".
  • Emergency Contact: Primarily for roleplaying purposes. This is the character to contact in the event the patient is severely injured, sickened, or if the patient expires.
  • Additional information: Enter in any other relevant medical information. This may include things such as disabilities or other disorders, be it mental or physical, or family medical history.. Other information which may fit here includes religious leanings and funerary arrangements, the executor of their will, or special requests from the patient.

Please be sure to place your entry in alphabetical order, according to the surname of the patient. Any fields that are left blank should contain "N/A".
